In a detailed letter, please include the following information:\u00a0<\/p><ul><li>Your name, civic address, \u00a0telephone number, email address<\/li><li>The date, time and location of the event<\/li><li>The circumstances surrounding the incident<\/li><li>A description of the damages<\/li><li>When possible, an estimate of the amount of the damages.<\/li><\/ul><p>The notice of claim must be sent to the Town Clerk within 15 days of the event, in conformity with Articles 585 to 590 of the Cities and Towns\u2019 Act.
